HomeSteady Health is a Direct Primary Care (DPC) practice. DPC is an innovative healthcare model where providers partner with their patients to provide the full spectrum of primary care services under a low monthly membership fee. Without the obstacle of insurance payments we can eliminate the administrative and logistical burdens that are roadblocks to connecting with your provider. Through transparency, cost effectiveness, personalized service and unprecedented communication we can work to restore the bond between patient and provider.
Our relationship begins with a free consultation to discuss membership plans, services, expectations and to ensure DPC is a good fit for you and your health needs. After membership is initiated, the initial visit will be an hour long visit in your home where we will discuss your full health picture, lifestyle, living environment and complete a physical exam. Visits after that will be conducted as you need them over the phone, via telemedicine, in the office, or at your home depending on the circumstances and your convenience. The provider is available 24/7 for additional communication, as needed.
How it works
Our rates
Membership
Members pay a low monthly fee which covers all available services with the exceptions of some medications and procedures. We do not accept insurance at this time, so there are no co-pays, coinsurance or additional fees. Insurance may be used for additional services like imaging, labs or medications.
Individual Membership
Our low monthly fees are tiered based on the age of the member with discounts available for families
3 to 18. $49.00
Ages 19 to 24 $74.00
Ages 25 to 58 $99.00
Ages 59 and up $129.00
Family memberships
Discounts are available for couples and children as you add members of your family to the plan.
Couple $174.00
Family (2 adults, 2 children) $249.00
Family (1 adults, 2 children) $174.00
Additional Children $26.00
Drop-in rates
You must a be a member to obtain services from HomeSteady Health. However, initial problem focused visits may be eligible for a drop in rate on a case by case basis. The drop in rate is $165 plus additional charges for procedures, medications, etc. if needed.
