Living with diabetes, high blood pressure, high cholesterol, or a thyroid condition is easier when one provider knows your whole story. At Homesteady Health, chronic condition management is ongoing, coordinated care from a single Nurse Practitioner who tracks your labs and your progress over time, in person near Ithaca or by telehealth across New York.
Serving Trumansburg, Ithaca, Lansing, Newfield, Enfield, Dryden, Groton, and the surrounding Finger Lakes communities.
Chronic condition management is the ongoing care of a long-term health condition: diagnosing it accurately, starting the right treatment, checking labs on a schedule, and adjusting your plan as your body and your life change. It is the difference between a single appointment and a relationship that follows your health from season to season.
Most chronic conditions, high blood pressure, type 2 diabetes, high cholesterol, and thyroid disorders among them, do not announce themselves loudly. They drift. Management is how you catch that drift early and keep it in a healthy range, so a manageable condition stays manageable. Homesteady Health handles that ongoing work inside a membership built for it, without the rushed visits and referral mazes that make chronic care harder than it needs to be.
Chronic condition management is ongoing, coordinated care for long-term health conditions: diagnosis, medication, regular labs, and monitoring, handled by one provider who tracks how you are doing over time rather than at a single visit.
Prevention is at the heart of chronic condition management. The conditions that do the most damage are usually the ones you cannot feel yet.
High blood pressure, high cholesterol, and early diabetes are insidious. They rarely cause symptoms until they have already done harm to your heart, kidneys, or blood vessels. Many people feel great and are certain nothing is wrong, while one of these conditions is quietly building under the surface. Feeling well is not the same as being well.
Many people feel perfectly fine and assume they do not need primary care. But high blood pressure, high cholesterol, and early diabetes are usually silent. Primary care is how you find and address them early, before they cause real harm.
That is exactly what proactive primary care is for. With the right labs and screening, and a provider who is actually looking, we can uncover these conditions, and others that hide, like sleep apnea, before they become a bigger problem. Caught early, most are far easier to manage or even reverse. That head start is the whole reason chronic condition management belongs in primary care, not in a visit after something has already gone wrong.
Homesteady Health manages the common, high-impact conditions primary care is built to handle, and coordinates with specialists when a condition calls for one.
A1c and continuous glucose monitoring (CGM), medication management, and the diet and activity coaching that keeps blood sugar steady. We catch prediabetes early, when it is most reversible with the right support.
Regular monitoring, medication tuned to your readings, and lifestyle guidance to lower cardiovascular risk. Hypertension is quiet, so a provider who tracks the trend matters.
Standard and advanced cholesterol testing, plus coronary calcium scoring to gauge your real heart risk, with statin or non-statin options when appropriate, reviewed and adjusted as your numbers move.
Lab-guided treatment for underactive and overactive thyroid, with unhurried dose adjustments. Learn more on our Thyroid Treatment page.
Because blood sugar, blood pressure, cholesterol, and weight travel together, we treat them together, with realistic, sustainable steps and GLP-1 medication options when appropriate. For hormone-related concerns, see our men’s and women’s hormonal care.
Managing something else long-term, from sleep apnea to another ongoing condition? We are the advocate in your corner, helping you navigate an ever-changing health care system and coordinating your medications and specialist visits so nothing falls through the cracks. See all of our services.
If you are managing an ongoing condition like diabetes, high blood pressure, high cholesterol, or a thyroid disorder, and you want it handled proactively rather than one rushed visit at a time, you qualify for chronic condition management at Homesteady Health.
Chronic conditions are managed best by one provider who knows your history, not a rotating cast you meet for ten minutes. You see Matthew at every visit, so dosing and lifestyle decisions get made with your whole health in view. That continuity is the heart of good chronic care.
Direct primary care is built around continuity and access, the two things chronic care depends on most.
You see Matthew every visit. Dosing and lifestyle decisions get made with your whole history in view, which is exactly what chronic care depends on.
Membership includes as many visits and follow-ups as your care needs, in person or virtual, with no copay at the door and time to actually talk.
Message your Nurse Practitioner directly when a reading looks off or a medication question comes up, instead of waiting weeks for a slot.
The regular lab work chronic care relies on, A1c, lipid panels, thyroid and metabolic panels, is available to members at discounted rates.
You see the same provider every time. That continuity is what makes the difference in chronic care: dosing and lifestyle decisions get made with your whole history in view.
We start with an unhurried visit: your history, your symptoms, your medications, and what you want your health to look like. No stopwatch.
We order the right panels for your condition, at discounted member rates, so decisions are guided by your actual numbers, not guesswork.
Together we set targets and a treatment plan, medication, lifestyle, and monitoring, that fits your life in the Finger Lakes, not a template.
We recheck labs, track your trend, and adjust as needed. The plan evolves with you, which is the whole point of management.
Living well with a chronic condition around Ithaca has a local rhythm to it. Our long Finger Lakes winters and short daylight months tend to quiet everyone’s activity and shift how we eat, and those seasonal changes can nudge blood pressure, blood sugar, and weight without anyone noticing until spring. Those same gray winters and springs also make vitamin D deficiency common here, something that is easy to overlook and simple to check with a lab. Year-round monitoring is how you catch that drift early rather than at an annual physical that is already months behind.
Care here is spread across Ithaca, Cortland, and the towns ringing Cayuga and Seneca Lakes, and juggling separate offices for every issue gets old fast. Having one provider who coordinates your conditions and medications, and who is reachable by telehealth from Trumansburg to Newfield to Groton, takes real friction out of staying on top of your health.
This is an active, outdoor place, and that is the point of managing a condition well: to keep hiking the gorges, working the land, and getting out on the lakes. Matthew lives the active Finger Lakes lifestyle himself and builds care plans meant to keep you doing what you love here.
Founder & Nurse Practitioner · Men's Health & TRT
Matthew founded HomeSteady Health to practice medicine without the clock and the profit-over-patient mindset of corporate care. Across more than 25 years he’s practiced family medicine in the Alaskan bush, worked trauma and the ER in fast-paced Philadelphia, and built urgent-care clinics there. Today he serves on the board of Region 2 of the Nurse Practitioner Association of New York, where he was named NP of the Year in 2025.
He also lives the active Finger Lakes lifestyle — running, grappling, basketball, coaching, hunting, fishing, and time out on Cayuga Lake. He understands the energy his patients are trying to get back because he depends on it too. Hormone treatment is medical care, not a product: it requires proper testing, thoughtful interpretation, and ongoing follow-up. That’s the standard every fatigue evaluation here is held to.
