I am sorry you are considering terminating your membership.  Please reach out if there is something we can do to make your experience better.  If you still wish to cancel, please see below from your previously signed member agreement for the steps needed to cancel your membership.  Once the formal letter is received your services will terminate - I am no longer involved in your care.  We will send you a formal termination letter.  

Quitting the practice: I acknowledge and understand that both HomeSteady Health Direct Primary Care and I have the absolute and unconditional right to terminate this agreement, without the showing of any cause for termination. I may terminate my agreement with HomeSteady Health Direct Primary Care at any time and for any reason by providing written notice to HomeSteady Health Direct Primary Care, 8500 Iradell Road, Trumansburg, NY 14886, and the agreement will terminate at the end of the current payment month.  I acknowledge and understand that HomeSteady Health Direct Primary Care may cancel this Member Agreement for any reason, including cause due to nonpayment of fees or for unruly, threatening, or inappropriate behavior, by providing me written notice. Any pre-paid monthly fees will be prorated from the date of cancellation and returned to me within ten (10) business days. HomeSteady Health Direct Primary Care will not terminate this agreement solely based on health status. HomeSteady Health Direct Primary Care will assist transferring records and care to the new primary care provider designated by the member. Any payments outstanding through the date of termination of the agreement are the responsibility of the member.

Rejoining: I acknowledge and understand that if I terminate my HomeSteady Health Direct Primary Care agreement after receiving initial services, I may reestablish my enrolled member status after payment of a $198 rejoining fee. I acknowledge and understand that HomeSteady Health Direct Primary Care is not obligated to allow me to re-enroll if I have previously terminated my agreement.
