Click the button to schedule your first Visit with HomeSteady Health. This visit is to take place in your home or at the clinic if you prefer. During this time I will get to know you and your full health picture.
2. Two days prior to your appointment you will be sent your intake paperwork. Please fill it out ahead of your visit and electronically sign the consents.
3. Look out for an invitation to Spruce. This app allows us to communicate securely through two way texting and phone calls. Download the app and sign on. Move it to the front page of your phone and turn on notifications.
4. Sign up for the patient passport (email sent from Elation) to receive your personal health information securely through our portal.
5. Submit your medical records from your other providers. You will need to send them a release of information form and make sure they fax it to 607-270-1277 (see member’s page for full details)
6. Receive the best healthcare in the region!
